HRT is a form of hormone treatment that transgender individuals typically undergo prior to gender-affirming surgeries. While Chinese law does not prohibit transgender individuals from using the basic social medical insurance system to access hormone therapy, in practice, hospitals or social security offices often deny coverage for medications like estradiol. They commonly cite reasons such as "male identity cannot use female medications" or "off-label use not specified in the drug instructions". This article is based on my experience in 2023 communicating with hospitals and the Shanghai Medical Insurance Bureau regarding reimbursement issues.
HRT (Hormone Replacement Therapy) 或者 GAHT (Gender-Affirming Hormone Therapy) 亦即激素替代治疗，是跨性别者在进行性别肯定手术前，通常会进行的激素治疗。中国法律并没有禁止跨性别者使用社会基本医疗保险进行激素替代疗法，但是在实际操作中，医院或者社会保障局会以“男性身份不能使用女性用药”或“使用说明未记载的用法”的理由拒绝跨性别者使用医保结算雌二醇等药物。这篇文章是我在 2023 年就医药报销问题与医院和上海市医保局沟通的经历。
